Does the engine have to be reprogrammed after changing the throttle body?

There are simple re-learn procedure that can be done recalibrate the idle speed and throttle learning. a popular one is to let the vehicle idle for 3 min, then ignition off 1 min, repeat 2 times. You should see your idle rpm's stabilizing around 600 rpm over the course of those 3 min. There are YouTube videos that explain this in depth.

No, mine didn't. Just replace it. Make sure all electric connection are secure and drive on.

No, if you installed the original part, which I had to do for good. Don't mess with that part, its a key component in the ignition system and its electronically controlled so you have to install the original part and no programming is required if you install the original part.
